Our Financial Policy
Thank you for choosing Laza Internal Medicine PLC for your healthcare needs. We are committed to providing you with high-quality medical care and services. To ensure a smooth and transparent financial experience for both our patients and our practice, we have established the following financial policy:
Insurance Coverage: a. We do not work directly with your insurance and we will not bill your insurance company for the services rendered. We will provide you with a superbill at the end of the visit in most instances or within 72 hours.
Full payment is due at the time of service unless other financial arrangements have been made in advance.
Payment Methods: a. We require all patients to have a valid credit card on file which will be charged at the time of visit or service rendered unless payment is provided with cash or checks at the time of the visit b. Returned checks will incur a fee, and future payments will be required in cash or via certified funds.
Outstanding Balances: a. Any outstanding balances must be paid within 30 days of receiving the statement
Missed Appointments and Cancellations: a. We kindly request at least 24 hours' notice if you need to cancel or reschedule an appointment. Failure to do so may result in a missed appointment fee. b. Multiple missed appointments or excessive cancellations may lead to the termination of your patient relationship with our practice.
Collections: a. In the event of non-payment or unresolved balances, we reserve the right to refer your account to a collection agency. Any additional fees incurred during the collections process will be your responsibility. b. We understand that financial difficulties may arise, and we encourage open communication to explore potential solutions.
Financial Responsibility: a. As the patient or responsible party, you are ultimately responsible for all charges incurred for services provided.
