Official Immigration Medical Examinations
We are excited to offer official immigration medical examinations at Laza Internal Medicine. We will work closely with you to streamline an efficient and expedient completion of the process. We are looking forward to helping you!
The medical exam consists of 3 major parts as follows:
A history, physical examination and mental health evaluation in person by the designated Civil Surgeon, Dr. Laza.
Mandatory bloodwork as per CDC guidelines. We will collect the bloodwork in the office and send it to Labcorp. We can submit your medical insurance information only for the bloodwork/urine samples. We also have available self-pay prices. Please call for information at (434)-261-1261.
Review of your vaccination records. Please bring all the vaccination records translated into English that you have available. If we determine that you need further vaccinations, we work with Giant Pharmacy located at 1900 Abbey Rd, Charlottesville, VA 22911. We will work with Giant Pharmacy to fulfill the vaccination requirements.
What to bring to your medical exam
Valid government issued ID such as a valid passport, US drivers license, green card or military ID.
Medical records translated into English including proof of prior treatment of tuberculosis, syphilis, gonorrhea, leprosy and prior chest x-ray reports.
Immunization records translated into English.
Any mental health records translated into English.
USCIS form I-693 with Part 1 pre-completed in capital letters.
**Note: Please do not complete Part 2 ahead of time**
If you need an interpreter, you must physically bring your interpreter to the medical examination. An interpreter will not be appointed by Laza Internal Medicine.
**Note: The blood/urine tests will be submitted to Labcorp along with your insurance provider to potentially get re-imbursed. However, Laza Internal Medicine cannot guarantee re-imbursement for this**
When can I receive my results?
If no other medical care or vaccinations are needed, you will be able to pick up your sealed envelope within 7 business days after your appointment.
