Official Immigration Medical Examinations

The Exam Includes:


History, Physical, and Mental Health Evaluation

  • Conducted in person by Civil Surgeon Dr. Laza

  • Fee: $250 (non-refundable and not covered by insurance; must be paid before the appointment)

  • Additional family members: $200 each

    Mandatory Bloodwork

  • Per CDC guidelines

  • Collected at the office and sent to Labcorp

  • Labcorp will bill separately (insurance may or may not reimburse)

Vaccination Record Review

  • Bring all available immunization records

  • Additional vaccines may be required

WHAT TO BRING TO YOUR APPOINTMENT



Valid Government-Issued ID

  • (e.g., Passport, U.S. Driver’s License, Green Card, Military ID)



    Translated Medical Records

  • Especially records related to: Tuberculosis, Syphilis, Gonorrhea, Leprosy, Chest X-rays

Translated Immunization Records

Translated Mental Health Records (if applicable)

◦ Include your name and alien number at the top of every page

DO NOT complete Part 2

Interpreter (if needed)

◦ Must be physically present
